2 Dead, 2 Seriously Injured in Crash that Started with Vehicle that Hit a Deer on I-90 WEST Near Hoffman Estates

An Illinois State Police trooper discovered a rollover crash about 1:45 a.m. Monday, May 31, 2021 at Mile Marker 59 on I-90 WEST (Jane Addams Memorial Tollway) near Hoffman Estates. The Illinois State Trooper discovered that the crash was likely caused by a deer that was found dead at the scene. A black 1992 Chevy Blazer was rolled over in Lane 1 on the left, and a 2006 Pontiac with severe rear-end damage was located near the right shoulder. Firefighters and paramedics from Hoffman Estates were immediately dispatched to the scene. The crash was reported near Beverly Road, east of Sutton Road, and was located right at the Mile Marker 59 sign and near the large Prairie Stone sign.




Hoffman Estate Fire Department responded with three ambulances. An Elgin Fire Department ambulance was assigned as the fourth ambulance. The Hoffman Estates Battalion Chief on arrival reported a vehicle on its roof on the left shoulder, and a vehicle on the right shoulder with major damage. The first patient extricated from the Pontiac by 2:05 by 2:05 a.m. was in traumatic cardiac arrest at the scene. A second victim was extricated from the Pontiac by 2:10 a.m. A third patient was also removed or extricated from the Pontiac.

The two passengers in the Pontiac were transported to Advocate Sherman Hospital in Elgin and Amita Health St. Alexius Medical Center in Hoffman Estates, where they were pronounced dead at each hospital. One of the passengers killed was a 15-year-old Chicago boy, who was identified as Carlos Fajardo. The other passenger killed was a 51-year-old woman. Initially her identity was not released.

The driver of the Pontiac, a 40-year-old woman from Chicago, was transported to St. Alexius hospital with serious injuries.

The driver of the Chevrolet Blazer, a 32-year-old man from Georgia was also transported to Sherman Hospital with serious injuries.




The preliminary investigation indicates that at approximately 1:45 a.m. Monday, the 2006 Pontiac was being driven westbound in the left center lane when the vehicle struck a deer. As the 2006 Pontiac slowed down, the driver of a Chevrolet Blazer SUV changed lanes onto the right shoulder and hit the rear end of the Pontiac.

Initially Lanes One and Two were blocked, but then all westbound lanes on the I-90 tollway, and the ramps from Sutton Road onto the tollway were closed for several hours for the accident reconstruction investigation.
