Lake County Sheriff’s Deputies and firefighters and paramedics from Lake Zurich responded about 9:35 a.m. Thursday April 8, 2021, at were dispatched to Route 12 and Deer Park Boulevard, Deer Park, for a traffic crash with injuries. Lake County Sheriff’s deputies and firefighters and paramedics from Lake Zurich arrived and located three vehicles with extensive damage. The three vehicles were a Peterbilt truck semi-trailer dump truck, a Chevrolet Venture minivan, and an Infiniti SUV. The dump truck was located almost one block east of the crash impact location near the former John’s Pub.
The male driver of a Chevrolet Venture minivan and a front-seat passenger — a 35-year-old woman of Champaign — were extricated and transported to Advocate Good Shepherd Hospital in unincorporated Barrington. Both victims in the Chevrolet Venture suffered with major injuries. The 35-year-old woman succumbed to her injuries at the hospital and was pronounced deceased. The driver remains at the hospital in critical condition.
The driver of a Peterbilt truck was transported to Advocate Good Shepherd Hospital with minor injuries. The driver of the Infiniti was not injured.
The Lake County Coroner’s Office responded, and an autopsy is being scheduled on the 35-year-old woman. The Illinois State Police and IDOT also responded to investigate, which is a standard procedure in fatal crashes involving commercial trucks.
FATAL CRASH INVESTIGATION
Preliminary investigation shows a 2000 Peterbilt Truck, hauling a partial load of gravel, was traveling southbound on Route 12. The Peterbilt was driven by a 65-year-old man of Melrose Park. The Peterbilt was in the outside lane of traffic approaching Deer Park Boulevard.
The male driver of a 2002 Chevrolet Venture Minivan was traveling southbound (US-12 EAST) next to the Peterbilt truck in the inside lane of traffic. The Chevrolet was being driven by a 36-year-old man of Harvey. For an unknown reason, the driver of the Chevrolet attempted to make a right turn onto westbound Deer Park Boulevard, from the inside lane of traffic. The driver of the minivan made a wide right turn directly in front of the Peterbilt truck. This caused the Peterbilt truck to strike the Chevrolet minivan on the passenger side as the minivan was turned right. The passenger that was killed was seated on severely damaged right side of minivan that took the impact of the semi-trailer dump truck. The Chevrolet minivan then struck a utility pole, before coming to a rest.
In an effort to avoid striking the Chevrolet again, the driver of the Peterbilt swerved and sideswiped a southbound 2018 Infiniti SUV, which was in the inside lane. The Infiniti was driven by a 64-year-old man of McHenry.
Following the crash impact, the Peterbilt dump truck was stopped further along US-12 EAST about 570 feet southeast of the Chevrolet minivan wreck at the southeast corner of Rand Road (US-12) and Deer Park Boulevard. A typical city block in the city of Chicago is 660 feet long.
The crash remains under investigation by the Lake County Sheriff’s Office Technical Crash Investigations Unit and Lake County Coroner’s Office.
